Office of the Assistant Drugs Controller

Drugs & Cosmetics Act 1940 and Rules 1945 was passed and implemented with the objective of making, standard quality drugs under fixed price, available to the people of India. In Kerala, State Drugs Control Department is authorised to implement the Act and Rules and proceedings related to it. The department functions under the state Health and Family Welfare Ministry. The Department Constitutes of State Drugs Controller, Deputy Drugs Controllers, Assistant Drugs Controllers, Regional Drugs Inspectors and Drugs Inspectors along with the required Ministerial Staff. Office of the Assistant Drugs Controller, Kannur functions under the Drugs Control Department with the jurisdictions of Kannur, Kasaragod districts. Storage, sale, distribution and quality implementation of Allopathic and Homoeopathic drugs come under this office. The Assistant Drugs Controller, Kannur is the Head of the office.

Implentation of the following Laws are the objective of this Office.

1. Drugs & Cosmetics Act 1940 & Rules 1945.
2. Drugs (Price Control) Order 1995.
3. Drugs & Magic Remedies ( Objectionable Advertisement) Act 1954 & Rules 1955.

Document required for Applications

1. For Allopathic Drugs’ Sales Licenses
2. A Covering letter duly signed by the applicant, affixed with a 5/- rupee Court fee stamp.
3. Form 19
4. Questionnire
5. Affidavit attested by Nottary, prepared in a stamp paper worth Rs. 100/-
6. E- chalan worth Rs.3000/-
7. Declaration of Registered Pharmacist in prescribed format in own hand writing.
8. Valid Pharmacy Registration Certificate
9. Purchase invoice of the Refrigerator
10. Request for Option
11. Copy of Partnership Deed/ Bye law/ Memorandum & articles of Association/ registration certificate of company/ Society if the applicant is a partnership/Co-operative/Trust firm
12. Self attested copies of Aadhar of the Applicant, Registered Pharmacist and partners of the partnership firms.
13. Self attested copy of PAN card of the applicant
14. 2 photographs of the applicant.
15. Rent agreement ( Agreement should be mutual. No sub lease agreement will be accepted. Building Number should be clearly mentioned in the agreement )
16. Ownership Certificate of the building ( issued for the purpose of obtaining drugs license)
17. Declaration of the Applicant ( Proprietor/Managing Partner )
18. Plan of the building with location

Requirements

1. The proposed Room should have minimum 10meter square area ( Around 110 Square feet)
2. Service of the Registered pharmacist is inevitable.
3. Almirah with glass shutters with 1.5 cubic meters storage spce ( Minimum )
4. Almirah with wooden shutters with 1.5 cubic meters storage space ( Minimum )
5. Refrigerator with a minimum capacity of 165 litres
6. The proposed room should have a separate door number
7. The proposed room should have separate electric connection.

 For Poison Permit / License

1. A Covering letter duly signed by the applicant, affixed with a 5/- rupee Court fee stamp.

4. Declaration of the Applicant
5. Declaration of the person in charge of poison ( the person should have passed S.S.L.C)
6. Aadhaar copy and photograph of the applicant and of the person in charge of poison

III.  For Approval of Competent Person

1. A Covering letter duly signed by the applicant, affixed with a 5/- rupee Court fee stamp.
2. Biodata of the proposed competent person.
3. Experince certificate.
4. E chalan worth Rs. 555/-
5. Form BB ( rearding the registration in Labour Department)
6. Declaration of the proposed competent person ( with Photo)
7. Original drugs licenses
8. Copy of the attendence register/pay vouchers
9. Copy of Aadhaar of the proposed competent person.

1. Endorsement of Registered Pharmacist

1. A Covering letter duely signed by the applicant, affixed with a 5/- rupee Court fee stamp.
2. E chalan worth Rs. 280/-
3. Copy of Aadhaar of the applicant and registered pharmacist
4. Declaration of the pharmacist
5. Original Drugs Licneses & Renewal Certificate / Retention letter.
